Instructions for Sample Submission Form

  1. Download the Sample Submission Form (SSF)
  2. Complete the form electronically in Microsoft Word to ensure legibility
  3. Print and include a signed copy with your shipment
  4. Email the completed SSF to [email protected] prior to shipment
  5. Notify us of shipment details, including tracking number and expected delivery date
